Forms

Many of the forms are in Adobe® Acrobat® file format. You need Adobe® Acrobat® Reader to see or print it.

Some of the forms are *.html documents that will open in , and can be printed from your browser. Please, note: if the forms do not show up well in your browser, or do not fit on your printer's page, (as if linefeeds and tabs are all wrong), check the font size specified as the default in your browser. A font size of 12 (Netscape Navigator) or "smallest" (Internet Explorer) should print OK. If there are still problems, consider changing the percent size you are printing. You may get everything to fit on the page if printed at 80% or so.
